UGMAC-2026: Mandatory Check Slip & Document Verification List

Every candidate participating in UGMAC-2026 counseling must bring original certificates along with self-attested photocopies in the following order:

  1. Matriculation (Class 10): Original Admit Card, Original Marksheet, and Original Passing / Provisional Certificate.
  2. Intermediate (Class 12 / I.Sc.): Original Admit Card, Original Marksheet, and Original Passing / Provisional Certificate.
  3. NEET (UG)-2026: Original Admit Card (Candidate Copy).
  4. NEET (UG)-2026: Original Score Card / Rank Card (Candidate Copy).
  5. Photographs: Exactly 6 copies of passport size photographs identical to the one used on the NEET (UG)-2026 Admit Card.
  6. Caste & Reservation Certificate: Original Certificate issued by Circle Officer (CO) / Revenue Officer (RO) of permanent residence.
  7. EWS Certificate: Original Income & Asset Certificate for Economically Weaker Section issued by DM / SDM / CO.
  8. Residential / Domicile Certificate: Original Permanent Resident Certificate issued by CO / RO. Parents' Employer Certificate where applicable.
  9. Character Certificate: Original Character Certificate issued by the Head of the Institution last attended.
  10. Disability Quota (DQ): UDID Card + Authorized Medical Board Disability Certificate along with Affidavit as per Clause 3.6 of the prospectus.
  11. NRI Quota: Valid requisite NRI claim certificates as per Clause 6(xi)h of the prospectus.
  12. Ward Quota (B.V.Sc. & A.H. / B.F.Sc.): Valid Ward Quota Certificate from competent authority as per Clause 5.3(c).

UGMAC-2026 Biometric Identification Report Guidelines

  • Candidate Credentials: The form requires Candidate Name, NEET Roll No, All India Rank (AIR), NEET Registration No, UGMAC-2026 ID, UR Rank, Category Rank, RCG Rank, and DQ Rank.
  • Biometric Capturing: Left Thumb Impression (LTI) / Right Thumb Impression (RTI), live facial photograph, and signatures are recorded on the spot before the verification team and technical agency (SYSTOMAT Solutions).
  • Identity Match: Photograph on NEET admit card, matric certificate, and identity marks will be verified. Any mismatch will lead to immediate disqualification.
